AACE REGISTRATION
To Register with AACE:
- Self-identify with AACE by completing and returning the Registration Form.
- Submit appropriate disability documentation that meets the University System of Georgia disability documentation guidelines.
Appropriate disability documentation includes the following components:
- A diagnostic statement based on the most current Diagnostic and Statistical Manual of Mental Disorders (DSM) and/or International Classification of Diseases (ICD) should be included, unless the evaluator is unable to do so due to school system regulations.
- Evaluators must be qualified professionals whose training and credentials are consistent with expertise in the disability for which they provide documentation and/or eligibility classification under the IDEA/Section 504.
- Evaluating professionals should include their name, signature, title, identifying credentials (e.g., license numbers), and contact information.
- Dates of evaluations must be included.
- The substantial limitation in a major life activity should be described.
- Demonstrates substantial limitations impacting performance in the academic environment, including a description of the expected duration, frequency, severity, and progression of the condition (if applicable).
- Accommodation recommendations made must be supported by a rationale that explains the necessity based on the student’s measured impairments and current substantial limitations.
- Documentation of accommodations approved in the past is encouraged but does not guarantee approval at the postsecondary level.
Examples of Disability Documentation:
- Educational, psychological, or medical records.
- Reports and assessments created by healthcare providers and psychologists. Supplemental documentation may include 504 plans, and IEP’s, but additional documents may be required.
- Documents that reflect education and accommodation history, such as Audiology Reports and Vision Assessments.
- Statement from a healthcare or other service profession.
If you think you may have a disability, but do not have a formal diagnosis, we may provide resources for obtaining a formal evaluation, and under some circumstances, we may be able to authorize provisional accommodations for one semester while we await documentation; particularly when it may be difficult to acquire documents from your provider in a timely manner. A copy of your most recent Individualized Educational Plan (IEP) or 504 Plan may also start the process. You can submit documentation when you are completing the registration form.

Regents Center for Learning Disorders
Students who are unsure as to whether or not they have a learning disability may seek assistance from Georgia State’s Regents Center for Learning Disorders (RCLD). Follow these steps to be tested:
- Self-identify with AACE by completing and returning the Registration Form.
- Students who choose to be tested at the RCLD should indicate that to their Access Coordinator that will assist them in the completion of an RCLD evaluation request packet.
- Complete the RCLD packet in its entirety. The packet includes the instructions for completing it.
- The cost of the evaluation is $500. As a deposit, include a check for the first half, $250, payable to Georgia State.
Under some circumstances, students may be able to receive provisional accommodations while we await the RCLD evaluation.
Once the completed packet has been returned, AACE will submit it to the Regents Center for Learning Disorders. A representative from the Regents Center will review the packet and contact the student to schedule an appointment.
Complete and current documentation is required as part of the request for accommodations. The University System of Georgia disability documentation guidelines and forms will help ensure that students provide appropriate documentation of their disability. Students can either give the guidelines to their health care provider to use for drafting a letter documenting their disability or give them the form to complete instead. Note: Only one of the two forms of documentation is required, not both.

Temporary Impairments
The Access and Accommodations Center (AACE) recognizes that individuals with temporary impairments due to injuries, surgery or short-term medical conditions (e.g. broken limbs, hand injuries, mobility limitations during recovery from surgery) may benefit from temporary campus assistance, support services and resources.
Request Temporary Assistance
To request temporary assistance from AACE, students must submit a Registration Form together with supporting medical documentation. Prescriptions will not suffice for this purpose.
Duration of Temporary Assistance
Temporary assistance will set to expire based on the expected duration of the limitation documented by the qualified medical professional. AACE may request additional documentation to verify the need for continued services after the estimated duration of the condition has expired.
Temporary Assistance Appointments
AACE will schedule an orientation appointment with student who has requested Temporary Assistance once all required documentation has been received. Students will be contacted by AACE to schedule an initial appointment. The meeting will include a discussion of students’ temporary assistance needs and available support services and resources.

Georgia State does not issue disabled parking placards. Students who are interested in obtaining a disabled parking permit must apply for one through the state. If the student is unable to use the campus transit (bus) system, they may be eligible for MARTA Mobility. - University Housing (on-campus student housing)
University Housing residents who have housing needs due to a temporary impairment should complete the Temporary Assistance Form and submit appropriate documentation. Requests to modify room assignments are subject to space availability.
